■ For membership registration, enter the following four items as shown in the 'Academy Registration Certificate' to register as a member. (1.2.3,4 questions)
Enter the name of the relevant foreign language instructor as a name registered with the local education office.
1. Academy registration number (No. 0000)
2. Name of academy
2. The name of the head of the academy or the name of the corporation where the instructor works (individual business entity: director name corporate business entity: corporate name)
3. Name of the relevant foreign instructor (name registered with the Office of Education)

You must enter the above information to obtain approval for membership registration and proceed with the training, print out the training certificate and keep it for one year.
